The only camera I could find a paper trail for reuse on a later mission is the Apollo 10 CSM Westinghouse colour wheel camera which was later used on the Apollo 12 CSM. Incidentally, the Apollo 14 LM carried a backup B&W camera for use in the event of a burn-out (ala Apollo 12) so that at least video downlink could be maintained with only minor interruption.

What modifications were needed to operate the camera in space?
According to the Apollo 12 mission reports:
(1) Painted it white for thermal control.
(2) Substituted coated metal gears for plasted gears in the color wheel drive mechanism.
(3) Provided heat conduction paths to the camera outer shell for radiation.
(4) Used a special bearing lubricant.
Why didn't they use the Apollo 11 CM color camera and modify it instead of the Apollo 10 CM camera?
Because they used the Apollo 11 CM camera in the Apollo 12 CM. They the used Apollo 10 CM camera on the Apollo 12 EVA.

My apologies, Joe is correct, the Apollo 10 CSM Westinghouse TV was modified and used on Apollo 12 on the lunar surface.

Some updated news: the Apollo 10 color camera was the only camera that was reused on a second Apollo mission. Those involved in camera development could not recall which camera was used in the Command Module on Apollo 12 but they didnt believe it was a reuse of the Apollo 11 Command Module camera. A the time there were only two cameras in addition to the Apollo 10 camera.
The reuse of cameras did not start in earnest until Skylab.
